What is First Link™?

First Link™ is a program that connects individuals and families experiencing dementia to opportunities for learning, community services and support.

If you or your family member has been diagnosed with Alzheimer's disease or a related dementia, then First Link™ is for you.

  • First Link™ gets you connected to support early and will support you throughout the course of the disease.

  • the more you know about Alzheimer's disease or a related dementia, the better you may be able to cope; and

  • you will be linked to community supports and services.

Who can make a referral to the program?

A referral can be made by anyone (self referral, family physician, community care partner etc.)

How do we make a referral to the First Link™Program?

It’s simple – just fax or e-mail a referral form and First Link™ will take it from there.
